ALL IVA inspectors must only follow the wording of the manual. In relation to the fog light issue, the wording could be better but the important point to note is that you only need an audible warning or reset function IF the fog comes on with side lights. It would help if the wording in the manual had an underlined or highlighted MAY in the first line below;
Note 4: Either of the following applies: 1) The rear fog lamp(s) may continue to operate until the position lamps are switched off, and the rear fog lamp(s) must then remain off until deliberately switched on again or, 2) An audible warning, additional to the mandatory tell-tale light, must be activated if the ignition is switched off or the ignition key is withdrawn and the driver's door is opened, whether the lamps in (RS4) are on or off, whilst the rear fog lamp switch is in the "on" position.

the first one is fine - it's point 2 (doesn't make note of any other lamps but the fog, in conjunction with ignition)

you may ignore point 2 as point 1 says "MAY" and therefore you do not need to complicate the system to switch on a rear fog light. If you require the fog to come on with the sides (position) then you will need to follow point 2 unless you have a fog switch that drops its latch when the headlights are turned off. It makes sense as otherwise you get out of the car in fog, get in the next evening and blind people behind when you put your sides on.

Where does it say the rear fog can come on with side lights? I can only find reference to the front fog lights.

I'll be honest I've not tried it yet will report tonight! Just wired it all up and to get the rear fog on after the 'module' resets it you have to switch the rear fog switch 'OFF' then 'ON' then all is solved! I'll be using this as it meets all the requirements as I understand them whether it's required or not!

Toed had suggested that I take the power from the headlight motor so with my meter I worked out that the red yellow is only live with dipped and main beam and no power with side lights on. So I have bought a rear fog that lights up. Do I simply spice into this red yellow and run it to the switch via a 5 amp in line fuse. Do I need a seperate live to make switch light up?

No, you don't need another supply, unless you have a fancy fog light switch that illuminates with the side lights. I was able to bridge from the switched supply (from the switch to the fog) to the lamp in the switch - 10 mins of trial and error...

Stiggy was right! Just had an email back from VOSA - it's poor manual working, as logn as the fog lights don't come up with the sides then you're OK .... As for updating the manual: Harumph. Out of curiousity, I checked to see what they are doing on the 'other' IVA class manuals that have been updated since - and I found this in the N2/N3 manual (https://www.gov.uk/government/uploads/system/uploads/attachment_data/file/354075/iva-inspection-manual-vehicle-categories-n2-and-n3-hgvs-2014.pdf): If this is where they are going, it'd tie up with the email.

Shouldn't be hard to wire up an buzzer when ignition off & fog light switch is on.

I hope we are all clear on this point now, it will come up again and we do try to keep our cars as easy to build as possible. So just to clarify, rear fog on with dip and main beam and not sides/position, switch that lights up and has a rear fog symbol on it or next to it. Many thanks to everyone for your help and understanding. The wording in the manual could be much better. No buzzer required or special wiring or fancy switch or complex relay.
